What To Bring To Your Appointment

How to make an appointment at the office of

Our office is open Monday through Friday.  Patients are seen by appointment only. We ask you call our Main Office number (937-436-5763) or CLICK HERE to make an appointment.  A member of our staff will request information about you and your orthopaedic problem.  We will also request insurance information, so it is helpful to have this information available when you call.

If you find it necessary to cancel an appointment, please do so within 24 hours of your appointment to avoid a late cancellation fee.  This will allow us to provide an opportunity for another patient to be seen.  Thank you for your understanding of our office procedures.

What to bring to your appointment:

• Completed patient information form
• Insurance card(s)
• Co-payment required by your insurance
• Referral authorization (if your insurance requires)
• X-rays, MRI films, or other diagnostic studies you may have had for this problem, including the written report of the results
• A list of medications you are currently taking
• Shorts (if a lower extremity or back problem)
• Tank top or sleeveless top (if an upper extremity problem)
